Transition Planning

Why is Transition Programming Important?

Transition Services provide a coordinated set of activities that are:

  • Designed within a results-oriented process
  • Focused on improving academic and functional achievement
  • Promoting movement from student to post-graduation employment and/or activities

Post-school Activities:

  • Post-secondary education
  • Integrated employment
  • Vocational education
  • Adult services
  • Continuing & Adult education
  • Independent living
  • Community participation
  • Self-Employment/Micro-enterprise
  • Day Habilitation/Programming

Transition-age Students:

  • The Autism Model School currently has 36 students enrolled in their transition program. (ages 14 – 21)
  • Students vary in their functioning levels and need different amounts of behavioral support to function in
    the workplace.
  • Several students are currently looking for sheltered, supported and competitive employment opportunities.
  • Several students are investigating micro-enterprise projects.
  • Five students that are planning on attending a university or college upon graduation.
  • Three students who are interested in joining the military
    upon graduation.
  • Students can attend the Autism Model School until their
    22nd birthday.
