The Autism Model School accepts applications for students in grades K–12 for enrollment based upon submission of the information listed below:

2. Academic Documentation
During the admissions process, we work with families to place students in the appropriate classroom/programs by reviewing provided academic information. Prior to beginning this process we request that families collect the most recent versions of the following documents, as applicable:
- Individualized Education Plan (I.E.P. or 504)
- Multi-factored Evaluation (M.F.E.)
- Immunization records
- Any other evaluation, assessment or report regarding student ability or needs.
3. Age and Residency
In order to be admitted into the Autism Model School, a student must be between the ages of 5-21 and be a legal resident of the state of Ohio.
